Physical Examinations 

Please print the Pre-Participation Physical Health History, Supplemental History, Physical Examination, Clearance, and Health History Update forms and have them completed and returned to the nurse's office by the indicated due dates in order for your student to be cleared for the season. All forms must be appropriately signed, dated, and stamped where indicated to be accepted. Please note that the Health History Update (HHU) is due every season. Thank you in advance!
